OMMP Extended Evaluation Doctor Visit

Looking to obtain or renew your OMMP card but do not have current medical documentation of your qualifying medical condition? One of our qualified doctors can provide a medical evaluation for patients 18 and older. All patients under 18 years of age must provide medical documentation of their qualifying condition. All doctor appointments are secured with a $50 deposit at the time of scheduling, while the remaining balance is due on the day of your scheduled appointment. Contact us for more information.
